      目的 提高对垂体腺瘤的生物学侵袭性生长特点与CT 和MRI表现的认识。方法 进行420 例垂体腺瘤的CT 和MRI检查与手术病理对照分析。结果 420 例中,鞍内突入蝶窦、鞍上池及第三脑室生长380 例,鞍内及鞍上向鞍旁海绵窦包绕颈内动脉,延伸至颅中窝,向额部、斜坡匍匐侵及生长40 例。形态:圆形、花生状、不规则形、哑铃状或腰鼓状。CT 等、高密度及显着增强占79.9%,混合密度伴囊性低密度、环行增强占20.1% ;MRIT1 WI等信号,T2 WI高信号、等信号、混合信号占92.3% ;T1 WI和T2 WI高信号占7.7% ;提示瘤卒中及坏死囊变73.5%,肿瘤大小1cm ×0.6cm ×0.8cm ~7cm ×5.2cm ×6cm .病理诊断:嫌色性者213 例,混合性者118 例,嗜酸性者75 例,腺瘤(未分类)12 例,恶性者2 例。结论 CT 及MRI提供的垂体瘤的整体形态与周围解剖关系等信息,对临床治疗方案制定和预后判断起到重要作用。
英文摘要:
      Objective To improve understanding of biologic invasive growth character and CT or MRI features of pituitary adenomas.Methods CT and MRI features were analyzed in 420 cases with pituitary adenomas verified by operative pathology.Results Of 420 cases the tumor developed from endosella into sphenoidal sinus,parasella,suprasella cistern and the third ventricle in 380,from endosella and suprasella into laterasella cavernous sinus surrounding internal carotid artery and into middle cranial fossa,slope serpiginosum of frontal part in 40.Shape of tumor was cycle,peanut like,irregular,collar bullon or lumbodrum like.In CT isophknic or high density and homogen ous enhancement was 79.9%.Mixed or cystal low density and cycloid enhancement was 20.1%.In MRI isosignal in T1WI and high signal or isosignal,mixed signal in T2WI wre 92.3%;high signal in T1WI and T2WI was 7.7%.It meant 73.5% of cases had tumor apoplexy and necrotic cyst.Size of tumor was 1cm×0.6cm×0.8cm~7cm×5.2cm×6cm.Pathological diagnosis as follows:the chromophobe in 213,the mixed in 118,the acidophilic in 75,adenoma(non classification)in 12 and the malignant in 2.Conclusion Message of the features and peripheral anatomical relationship on pituitary adenomas was provided in CT and MRI,which played an important role in deciding clinical therapy and prognosis.
