| 目的探索股骨头缺血坏死(AVN)介入治疗可行性的客观依据。方法取健康成年新西兰兔20只,随机分成三组：A组(正常对照)5只；B组(模型组)5只,C组(治疗组)10只。将溶栓药物、扩血管药物和改善微循环药物直接注入股骨头供血动脉,借助经腹主动脉灌铅、MRI和组织学检查,对比观察了三组各项指标的变化。结果①和对照组比较,模型组股骨颈、干T2WI信号增高,尤以STIR明显,治疗组股骨颈、股骨干MRIT2WI及STIR序列高信号基本恢复；②和模型组比较,股骨头供血动脉数目有明显增加(P<0.001);③股骨头空缺骨陷窝数明显减少(P<0.005),软骨下血管数量及血管直径明显增加(P<0.05).结论介入溶通疗法可改善激素性缺血股骨头的血供,对早期股骨头缺血坏死是一种有效的治疗方法。 |

| Objective To find the experimental evidence for interventional therapy used in avascular necrosis of femoral head.Methods 20 rabbits were randomly divided into 3 groups:Group A(normal control),B(model), and Group C(treatment).On the basis of hormone induced AVN model,Urokinase,Dextran and injectio salviae miltiorrhizae were infused into the supplying arteries directly.MR signal,the number of supplying artery,and the histological changes were observed and compared between each group.Results ①Compared with control group,the MRI T2WI signal in model group became higher,especpally in STIR sequence and nearly return to normal in group C.②The number of supplying artery in group C increased with great significance compared with group B ( P <0.001).③The number of empty lacuna is decreased ( P <0.005),while the subchondral vascular number in group C is increased with great significant difference ( P <0.05) compared with group B.Conclusion Interventional therapy can improve the blood supplying of hormone induced AVN,which is an effective method in the treatment of early AVN of the femoral head. |
