| 目的探讨盆腔血池显像(PBI)对盆腔静脉淤血综合征(PVCS)的诊断价值,建立显像诊断标准。方法健康妇女30例,年龄27～48岁,观察组130例,年龄28～49岁。于月经干净后1周左右,静脉注射99mTc PYP740MBq,30,60,90,120分钟,应用SimensSPECT,配以低能高分辨准直器,矩阵64×64,采集盆腔前位,后位各2分钟记数,勾画盆腔静脉丛感兴趣区和髂血管区感兴趣区,求出各自记数/象素(count/pixel)进行比较,求前、后平均数,以x±s表示,大于两个s为异常。结果以腹腔镜为金标准,发现PVCS组各时间点比值均大于对照组(P<0.05),轻度淤血各时间点比值分别为：0.59<0.71,0.72<0.85,0.79<0.97,0.67<0.88,其灵敏度,特异性,准确性分别为90.5%,92.6%,93.2%,中度淤血各值为：0.72<0.83,0.86<0.98,0.98<1.15,0.89<1.09,93.8%,94.2%,94.8%,重度淤血：0.84<,0.99<,1.16<,1.10<,97.8%,92.4%,96.3%. |

| Objection To demonstrate the value of SPECT pelvic blood image(PBI) in diagnostic pelvic venous congestion syndrome (PVCS).Methods Health women 30 cases,25～47 years old as contrast group.Observe group was 130 cases women,27～48 years old who were suspected of PVCS.Aforesaid women were injected 99mTc PYP740 MBq after monthly 1 week,following 30 60 90 120 minutes those pelvic ANT and POS T were collected 2 minutes separately by Simens SPECT with low energy high separate collimator,matrix 64×64.then made ROI within pelvic venous region and bigvenous region to gain the between ratio of counts/pixel.All data expressed as ±s .Celioscope conceived of as gold standard compare with PBI to demonstrate its diagnostic value.Results The ratio of count /pixel was higher in PVCS group than in constrast group ( P <0 05).PBI exam were quite sensitive,its sensitivity,speciality and accuracy was 94 2%,93 4% and 96 4%.PBI has high relativity determinating degree of PVCS with celioscope.In PVCS 1degree,2degree,and 3degree,ratio of count/pixel was separately 0 59<0 71,0 72<0 83,0 84<at 30 minute,0 72<0.85,0 86<0 98,0 99<at 60 minute,0 79<0 97,0 98<1 15,1 16<at 90 minute,0 67<0 88,0 89<1 09,1 10<at 120 minute.Those sensitivity was 90 5%,93 8% and 97 8%.Speciality was 92 4%,94 2% and 92 4%.Accuraty was 93 2%,94 8% and 96 3%.Conclusion PBI is a safest,accurate,impersonal,untraumatic and repeatable methods in diagnosis PVCS. |
