| 目的探讨二维及彩色超声多普勒新技术在结肠癌诊断上的实用价值。方法收集应用二维超声及彩色多普勒血流显像诊断结肠癌23例。全部病例均经手术及病理证实,超声诊断符合率100%.结果声像图可将结肠癌区分为肿块型、壁增厚型和溃疡型三种。在壁增厚型中内膜明显凸凹不平,并见限局性固定强回声,结合患者便血,可诊断溃疡型。三种类型瘤体内部血流彩色多普勒信号的丰富程度及收缩期最高峰值流速,舒张末期最低流速均有一定差异,而RI值却无显着差异。结论二维及彩色多普勒超声对结肠癌诊断及鉴别诊断有重要的价值,提高对进展期结肠癌的敏感性,结肠癌均有不同丰富程度的彩色血流信号。超声可以配合纤维结肠镜及X线钡灌肠诊断结肠癌,还能帮助了解病变外浸润和转移情况。 |

| Purpose Evaluation of Two dimensional Ultrasonography(2D) and Color Doppler Flow Imaging (CDFI) in diagnosis of colon carcinoma. Methods 2D and CDFI were performed in 23 patients with Colon Carcinoma confirmed by operation and pathology.The diagnostic accuracy by ultrasound was 100%.Results It was proposed that the ultrasonograms of colon carcinoma may be divided into three types:the mass type, the thick wall type and the ulcer type. In the thick wall type, if intima is very irregular and focal areas are showed bright echogenicity combining with hematochezia we can diagnose the ulcer type. We found significant differences in CDFI, Vmax and Vmin between the mass type and the thick wall type, but no significance in RI Values. Conclusion 2D and CDFI are significant in diagnosis and differential diagnosis of colon carcinoma. It can improve sensitivity of the developing colon carcinoma. Colon carcinoma presents rich blood flow. Ultrasound can make up the weakness of colonofibroscope and barium enema being unable to peep at the depth of the pathological changes and the insufficiency of the extra intestine manifestations of the carcinoma. |
