| 目的本文探讨经内镜注射治疗Dieulafoy病大出血。方法Dieulafoy病是上消化道出血的罕见病因。自1992年2月至1997年3月经胃镜诊断上消化道出血1240例,检出Dieulafoy病13例,占1.0%.本病以呕血和黑便为主要表现,易发生失血性休克。病变部位多见于胃体近侧端,表现为病变中心小动脉喷血或血管断端渗血,13例患者除1例手术治疗外,12例于血管周围注射5%鱼肝油酸钠。结果10例即刻止血,有效率83.3%,治疗后无严重副作用发生。 |

| Aim Purpose of is to inquire into the endoscopic treatment of acute upper gastrointesinal bleeding due to Dieulafoy’s disease.Methods It was found in 13 of 1240 patients with upper gastrointestinal bleeding seen by us from February 1992 to March 1997.The most common clinical symptoms were hematemesis and melena,8 patients had hemorrhagic shock.The lesions presented as a protruding vessel with active bleeding including oozing and sourting of blood .Emergency therapy was endocopic injections of 5% varicocid into four to six spots round the lesion .Results 10 patients was successful (83.3%) by injecting 5% varicocid into the lesion,There were no endoscopic complications.Conclusion 5% varicocid injection under endoscopy is safe and effective,it also offers a new method of treatment to Dieulafoy’s disease. |
