      目的:评价磁共振成像在胰腺肿瘤诊断的价值。方法:本文通过对25例有手术病理的胰腺病变进行MR检查, 采用自旋回波T1和T2加权像对病变进行判断, 并与CT和B超对比分析。结果:这三种影像方法对胰腺癌的诊断准确率均为72.7%.胰腺癌的MR表现, 无论在T1还是T2加权像上其信号改变无特点, 且78%的胰腺癌信号强度与正常的肝组织和胰腺接近, 故MR无法区别胰腺癌或胰腺其它病变。但在T1加权像上, 由于MR的组织分辨率高, 因而无论是显示胰周脂肪, 肠系膜上动静脉有无受侵, 还是肝门、脾门、脾静脉有无受侵均较CT显示更佳。

      Purpose:To assess the value of MRI in diagnosing pancreatic neoplasms. Methods:Twenty five patients with pathologically proved pancreatic carcinomas or other neoplasms were evaluated with MR images obtained with T1 weighted and T2 weighted pulse sequences. CT and Ultrasound were used as the reference standard to determine the ability of MR to delineate normal and abnormal pancreatic anatomy and thereby to detect or exclude pancreatic malignancy. Results:The diagnostic accuracy of all these three imaging methods(MRI,CT,Ultrasound)was 72.7%. The Changes of signal intensity in 78% pancreatic carcinomas were not significantly different between T1 weighted and T2 weighted images and were similar to normal hepatic or pancreatic tissues. Short TR/TE SE sequence was significantly better than long TR/TE SE sequence in resolution of both normal and abnormal anatomy. The superiority of MR over CT for pancreatic tumors tended to occur in large tumor, extent of infiltration of the peripancreatic tissues, superior mesenteric artery etc.,Conclusion:MR imaging have no significant advantages over CT for evaluating pancreatic neoplasms when we only use T1 and T2 weighted SE sequences. But it is more useful to detect extent of infiltration of the peripancreatic tissues, superior mesenteric artery than CT.
