| 目的:报告4例无肝外结核症状的局限性肝结核,对有关文献进行复习,讨论局限性肝结核的CT表现。材料和方法:报告的4例肝结核3例经手术病理证实,1例临床治愈。4例均为男性,平均年龄为42岁。两例为结核瘤,属于局限性肝结核的结节型；两例为局限性肝结核的弥漫型。结果:局限性肝结核的病灶多位于肝脏的边缘部,表现为单发或多发的局限性低密度灶,边界较清楚或模糊,密度均匀或不均匀,可依病变发展的不同阶段而伴有细点状或斑块状钙化,增强可以不强化、环形强化、或不均匀强化。结论:局限性肝结核的CT表现无特异性而是随疾病的发展阶段的不同而不同,由无钙化的低密度病灶,演变为伴有钙化的高密度病灶,其演变过程符合疾病本身的病理变化过程,即肉芽肿,坏死液化,钙化,纤维化等。因此本病的确诊需依据组织学和细菌学检查。 |

| Purpose:To report four cases of local hepatic tuberculosis without clinical evidence of tuberculosis elsewhere.Some papers about that were reviewed.To discuss the CT findings of local hepatic tuberculosis.Materials and methods:CT scan was performed in 4 patients with local hepatic tuberculosis.3 cases proved by surgery.1 case cured by antituberculous chemotherapy.All of them were males.Their mean age is 42 years old.Two cases were tuberculomas (Nodular form of local hepatic tuberculosis).Two cases were diffuse form of local hepatic tuberculosis.Results:Most of the lesions of local hepatic tuberculosis were found over the surface of the liver.The CT findings were single or multiple focal low density lesions,homogeneous or inhomogenous,with or without calcification,no enhancement or with peripheral enhancement or inhomogeneous enhancement after contrast administration.Conclusion:The CT findings of local hepatic tuberculosis are unspecific.They will vary from the development of the disease,from a noncalcified low density to a calcified high density.They accord with the changes of the diseases:granulomatous tissue,liquefaction necrosis,calcification and fibrosis.The correct diagnosis was made by histology and bacteriology. |
