      目的 应用极速脉搏波(UFPWV)技术探讨维生素D缺乏对2 型糖尿病(T2DM)患者早期颈动脉弹性功能的影响。方法:根据25(OH)D3的检测结果将入选的92例T2DM患者分为维生素D缺乏组和维生素D非缺乏组,应用UFPWV技术测量颈动脉弹性参数,包括收缩期起始时、收缩期结束时脉搏波速度(PWV-BS、 PWV-ES)。电子病例系统收集一般临床资料及实验室指标。对上述数据进行统计学分析。结果:①T2DM患者中,维生素D缺乏组的BMI、空腹血糖、收缩压、舒张压、总胆固醇、甘油三酯、PWV-BS、PWV-ES水平均高于非缺乏组(P<0.05),HDL-C水平低于维生素D非缺乏组(P<0.05)。②Pearson相关分析发现,PWV-BS、 PWV-ES与年龄、BMI、空腹血糖、收缩压、舒张压、总胆固醇呈正相关(P<0.05),与25(OH)D3、HDL-C呈负相关(P<0.05)。③多元线性逐步回归分析显示,空腹血糖、收缩压、年龄、总胆固醇是PWV-BS、PWV-ES的危险因素(P<0.05),25(OH)D3是PWV-ES的保护因素(P<0.05)。结论:维生素D缺乏的T2DM患者PWV-BS、PWV-ES值明显升高,维生素D可能是2型糖尿病患者颈动脉硬化的保护因素,对2型糖尿病患者进行维生素水平的监测并及时补充维生素D或许可以延缓其并发症的发生发展。

      Objective To study the effect of vitamin D deficiency on early carotid elasticity in patients with type 2 diabetes mellitus (T2DM) using ultrafast pulse wave velocity (UFPWV) technique. Methods According to the results of 25(OH)D3, 92 patients with T2DM were divided into vitamin D deficiency group and vitamin D non-deficiency group. UFPWV technique was used to measure carotid elasticity parameters, including pulse wave velocity at the beginning of systole(PWV-BS) and at the end of systole(PWV-ES). The general clinical data and laboratory indicators were collected from the electronic case system. Statistical analysis was performed on the above data. Results ① In patients with T2DM, the levels of BMI, fasting blood glucose, systolic blood pressure, diastolic blood pressure, total cholesterol, triglyceride, PWV-BS, and PWV-ES in the vitamin D deficiency group were higher than those in the non-deficient group (P<0.05). The HDL-C level was lower than the vitamin D non-deficient group (P<0.05). ② Pearson correlation analysis found that PWV-BS and PWV-ES were positively correlated with age, BMI, fasting blood glucose, systolic blood pressure, diastolic blood pressure and total cholesterol (P<0.05), and negatively correlated with 25(OH)D3and HDL-C. <0.05). ③Multivariate linear stepwise regression analysis showed that fasting blood glucose, systolic blood pressure, age and total cholesterol were risk factors for PWV-BS and PWV-ES (P<0.05), and 25(OH)D3 was a protective factor for PWV-ES (P<0.05). Conclusion PWV-BS and PWV-ES values in patients with vitamin D deficiency are significantly elevated, vitamin D may be a protective factor for carotid elasticity in patients with type 2 diabetes. Vitamin level monitoring in patients with type 2 diabetes is promptly supplemented. Vitamin D may delay the development of its complications.
