      目的 探讨18F-FDG PET/CT在小儿神经母细胞瘤(NB)中的应用价值。方法 回顾性分析20例经病理确诊的NB患儿的临床资料,对所有患儿均行18F-FDG PET/CT检查,测量病灶的最大标准化摄取值(SUVmax),采用秩和检验比较治疗前组及治疗后组SUVmax的差异;采用Spearman相关性分析评估神经元特异性烯醇化酶(NSE)与病灶SUVmax的相关性。结果 根据18F-FDG PET/CT及国际神经母细胞瘤分期系统(INSS)进行分期:8例治疗前患儿中Ⅱ期1例、Ⅲ期3例、Ⅳ期4例;12例治疗后患儿中Ⅱ期1例、Ⅲ期2例、Ⅳ期9例。治疗前组、治疗后组NB患儿瘤体SUVmax分别为4.73±;1.43、1.64±;0.67(W=78.500,P<0.001)。20例患儿NSE为(70.92±;85.61)ng/ml,SUVmax为2.88±;1.85,二者呈正相关(r=0.699,P<0.05)。结论 18F-FDG PET/CT可辅助对NB患儿进行临床分期、监测化疗前后瘤体性质、并判断合适的手术时机。

      Objective To explore the application of 18F-FDG PET/CT in pediatric neuroblastoma (NB). Methods A total of 20 cases diagnosed as pediatric NB according to pathological results were retrospectively analyzed. The patients underwent 18F-FDG PET/CT scan, and then the maximum standardized uptake value (SUVmax) were measured. The difference of SUVmax between prior-treatment group (n=8) and post-treatment group (n=12) was compared by Rank sum test, and the correlation between neuronspecific enolase (NSE) and SUVmax was analyzed by Spearman correlation analysis. Results According to 18F-FDG PET/CT and international neuroblastoma staging system (INSS), there were 1 case of stage Ⅱ, 3 cases of stage Ⅲ, 4 cases of stage Ⅳ in priortreatment group, and 1 case of stage Ⅱ, 2 cases of stage Ⅲ, 9 cases of stage Ⅳ in the post-treatment group. The SUVmax of prior-treatment group and post-treatment group were 4.73±1.43 and 1.64±0.67 (W=78.500, P<0.001). The NSE of total 20 cases was (70.92±85.61)ng/ml, and the positive correlation was found with SUVmax (2.88±1.85; r=0.699, P<0.05). Conclusion 18F-FDG PET/CT plays a significant role in the process of clinical staging of pediatric NB, the monitoring of tumor features before and after chemotherapy, and the judgment of timing operation.
